New patients will need to complete the new patient packet prior to seeing Dr. Fan. Please print and fill out the packet below. Alternatively, the packet is available in the office waiting room. Our staff are happy to help you with any questions you may have.
Returning Patients
Dr. Fan’s office utilizes Athena Health, a convenient online patient portal where you can schedule appointments, access forms, view billing and notes and even make co-payments online at your convenience. All patients have access to their records using a secure login.
